To the best of my knowledge, I am in good physical condition and fully able to participate in this
course. I am fully aware of the risks and hazards connected with the participation in Brazilian
Jiu-Jitsu, Krav Maga, Combat Kickboxing, Warrior Academy for Kids, Fitness Training, Weightlifting, or
Aerobics, including physical injury or even death, and hereby elect to voluntarily participate in
said event, knowing that the associated physical activity may be hazardous to me and my
property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OR
LOSS, PROPERTY DAMAGE, OR PERSONAL INJURY, INCLUDING DEATH, that may
be sustained by me, or loss or damage to property owned by me, as a result of participation in this
course. I further certify that I am at least 18 years of age. If under 18, my parent/guardian is the
below signed.

I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE, Krav Maga Roanoke, Roanoke Valley Academy of Martial Arts,LLC, RVAMA, LLC, Impact Fitness, their officers, servants, agents and employees(hereinafter referred to as RELEASES) FROM ANY AND ALL LIABILITY, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in physical activity, or while on or upon the premises where such participation in classes takes place. I understand that as part of participating in such classes I will be submitted(forced to tap) by means such as choking and joint locks. I understand I may have joints injured, limbs broken, and I could possibly be choked unconscious. 

It is my expressed intent that this release and hold harmless agreement shall bind the
members of my family and spouse, if I am alive, and my heirs, assigns and personal
representative, if I am deceased, and shall be deemed as a RELEASE, WAIVE,
DISCHARGE, and CONVENTION TO SUE the above named RELEASEES. I hereby
further agree that this Waiver of Liability and Hold Harmless Agreement shall be constructed in
accordance with the laws of the State of Virginia.

In signing this release, I acknowledge and represent that I HAVE READ THE
FOREGOING Waiver of Liability and Hold Harmless Agreement, UNDERSTAND IT
AND SIGN IT VOLUNTARILY as my own free act and deed; no oral representations,
statements or inducements, apart from the foregoing written agreements have been made; and I
EXECUTE THIS RELEASE FOR FULL, ADEQUATE AND COMPLETE
CONSIDERATION FULLY INTENDING TO BE BOUND BY SAME.

SOCIAL MEDIA RELEASE FORM

This Social Media Release Krav Maga Roanoke is executed on {sign_date}, by {name}. The Releasor, {name}, who acknowledges and agrees to the terms below:

The Releasor grants permission to Krav Maga Roanoke to use their image, voice, or likeness in photographs, audio recordings, or video recordings on the Releasee's, Krav Maga Roanoke and/or 1441 Jiu Jitsu's website and social media accounts, including, but not limited to, Facebook, Instagram, TikTok, and X (Twitter), without acknowledgment or recognition given to the Releasor.
The Releasor grants the Releasee creative permission to alter the photographs, provided that the photographs are not altered in an explicit manner or used to maliciously represent the Releasor or their associates.
The Releasor understands that they will not receive any monetary compensation from Krav Maga Roanoke for the permissions granted herein and hereby waives any right of inspection or approval of the photos or recordings prior to the Krav Maga Roanoke posting on their social media accounts.
The Releasor acknowledges that any third party (including any agency, client, publication, or other organization or institution) may distribute the photographs and recordings on their social media accounts for the purposes of publicity and promotion of the Krav Maga Roanoke.
In giving this consent, the Releasor releases Krav Maga Roanoke from liability for any violation of any personal or proprietary right the Releasor may have in connection with all third parties’ use of the images on social media.
The Releasor certifies that they are 18 years of age or older. If the Releasor is under the age of 18, their parent or guardian must sign this Release.

In witness whereof, the Releasor executes this Release by signing below.
